Case Study chapter 49 Patient with urinary disorders

Sue Meade 22 years of age presents to the clinic with complaints of burning pain and urgency when urinating. The patient has a fever of 100F. The urine is strong in odor and cloudy with sediment.

A. What questions should the nurse ask during the assessment to gain more information about the possible causes of the urinary tract infection? How much water/fluids do you think you drink in a day? Do you think you drink an adequate amount of fluids/ water? What other symptoms do you have? Are you taking any medication? Have you ever had a UTI before? Do you use contraceptive? What kind? Are you sexually active? Do you urinate after sex? How many times do you urinate in one day? Do you have any pain? Where is the pain located? B. What patient education should be provided? The nurse should educate on taking plenty of water daily, to flush out any bacteria from the system. Encourage patient to void as soon as possible after sexual intercourse, and to take antibiotics as prescribed.

Educate on proper wiping from front to back, encourage patient to void frequently. Teach about sign and symptoms of worsening UTI. Educate on drinking acidic juices can aid decreasing bacteria in the bladder by acidifying the urine.

John Lyons has undergone treatment for kidney stones three times in the last year. Today he calls this primary provider with complaints of acute, excruciating, colicky pain that radiates down to the thigh toward the genitalia. The pain started last night and has become progressively more severe. He also complains about the urge to void and has little urine output and it is blood tinged. He has not taken any fluids or food in 24 hours. Mr. Lyons undergoes lithotripsy and analysis of stone fragments to reveal calcium stones.

A. Mr. Lyons symptoms are most indicatives of stones in what area of the renal system? The symptoms of Mr. Lyons are consistent with ureteral stones.

Mr. Lyons has the following orders:

Morphine sulfate 2mg IVP every 2 hrs as needed for severe pain Ibuprofen 600ng every 6 hours Normal Saline at 200mL/hr B. What are the rationales for the above orders?
