Bipolar disorder - Mood swings

Mania - grandiosity/inflated self-esteem - Decreased need for sleep - More talkative - Flight of ideas/racing thoughts - Distractability - Psychosis (hallucinations &/or delusions) - Increased goal-directed behavior - Self-destructive behaviors

Mood swings from depression to mania - 2 levels: - bipolar - cyclothymia: same set of symptoms- psychosis (more mild than bipolar)

Causes for bipolar disorder - Biological factors: - heredity (8-9% risk of 1st-degree relative) - identical twin concordance = 60% - fraternal twin = 12% - pc;p gene plays a love in synaptic transmission

Brain abnormalities - Reduced blood flow in right prefrontal cortex - Enlarged amygdala: thalmus

Biochemical factors - Higher levels of norepinephrine dopamine, acetlcholine & glutamate - Lower levels of serotonin

Seasonal effect (possibly) - Pregnant mothers with flu especially during 2nd or 3rd trimesters

Bipolar - psychosocial - Stress complicates - Low social support complicates Bipolar - sociocultural factors - Lower in black, asian, and hispanic populations - Higher in white and native americans - No gender differences in prevalence, but - males - mania - females - depression

Bipolar treatment - Lithium - regulates glutamate - anti-convulsions - tegretol (carbamzepine) - pepakote (valproate)

  • anti-psychotics
  • cognitive - behavioral therapy
  • interpersonal and social rhythm therapy ( IPSRT)
    • ECT
