Chaffey College ADN Program Administering Oral/Topical Medications

FOR ALL PROCEDURES STUDENTS MUST: 1. Verify physicians order 6. Wash hands/use hand hygiene product before, 2. Identify patient during and after procedure as appropriate 3. Check for allergies (as appropriate) 7. Gather supplies (check expiration date) 4. Explain procedure to patient 8. Medications to have 3 checks of 5 rights and 5. Provide privacy check of expiration date.

Preparation Pass Fail/Comments Retake Compare physician orders with MAR Assess for any contraindications to oral meds, allergies, diet history, lab results Obtain medication (from med cart or med delivery system), assessing for correct med, client, time, dose, route (verbalize 5 rights) (Check #1) Perform calculations as needed for administration Prepare medication for one client at a time Place all meds in medicine cup, do not touch unpackaged meds (floor stock) with bare hands (Check #2) For liquid medication, pour into med cup with label against palm of hand, and cup on level surface at eye level If preparing narcotic, check MAR for time and dose of last administration Never leave drugs unattended Procedure Explain to client each medication and action. Answer any questions Open each med into medicine cup after checking 5 rights (Check #3) Assist client to sitting or side-lying position If client able, allow to hold liquid and medication cup For sublingual administration, have client place under tongue until dissolved For buccal administration, have client place in mouth against mucous membranes until dissolved Mix powdered medications with liquids at bedside Stay until client has completely swallowed each medication Return client to comfortable and safe position Remove equipment and discard trash Record administration of medication on MAR Return within 30 minutes to evaluate client’s response to medication
