Medication: What is it for? What is its action? Side Effects Nursing Implications (Vital signs, labs, assessments)

Administration Techniques (example: IV push)

Propofol

Sedation propofol may work by decreasing the dissociation of GABA from GABA receptors in the brain and potentiating the inhibitory effects of the neurotransmitter.

chest pain or discomfort, confusion, dark-colored urine, dizziness, drowsiness, lightheadedness, fainting, fever, muscle cramps, spasms, pain, or stiffness,

assess level of consciousness and identify early signs of hypotension, bradycardia, apnea, airway obstruction and/or oxygen desaturation

IV, IV Push

Dexmedetomidin e Dexdor, Igalmi, Precedex

an alpha-2 agonist used for sedation during various procedures

it inhibits the release if norepinephrine, therefore, terminate the propagation of pain signals.

Blurred vision, chest pain or discomfort, confusion, coughing, dizziness, cardiac arrhythmia

Assess heart rate, ECG, assess BP (hypotension), and SPO

IV

Fentanyl

opioid analgesic Fentanyl binds to opioid receptors, especially the mu opioid receptor, which are coupled to G-proteins

relaxation, euphoria, pain relief, sedation, confusion, drowsiness, dizziness, nausea and vomiting, urinary retention, pupillary constriction, and respiratory depression

Assess vitals, respirations, SPO2, and mental status

IV,IM , Patch

Dobutamine

decompensated congestive heart failure

Dobutamine's ionotropic effect increases contractility, leading to decreased end-systolic volume and, therefore, increased stroke volume

increased heart rate and increased blood pressure, ventricular ectopic activity, nervousness, N/V/D

Advise patient to inform nurse immediately if chest pain; dyspnea; or numbness, tingling, or burning of extremities occurs.

Continuous IV Dobutamine 12 mg/ml

Dopamine

Dopamine is a catecholamine neurotransmitter used to treat hemodynamic

Dopamine produces positive chronotropic and inotropic effects on the myocardium, resulting in increased heart rate and

lightheadedness, chest pain, fast, slow, or pounding heartbeats, shortness of breath,

Report any rhythm disturbances or symptoms of increased arrhythmias, including palpitations, chest discomfort, shortness

IV

Medication: What is it for? What is its action? Side Effects Nursing Implications (Vital signs, labs, assessments)

Administration Techniques (example: IV push)

imbalances, poor perfusion of vital organs, low cardiac output, and hypotension.

cardiac contractility. of breath, fainting, and fatigue/weakness.

Norepinephrine

used to raise blood pressure in patients with severe, acute hypotension (short- term low blood pressure)

predominantly stimulates α 1 receptors to cause peripheral vasoconstriction and increase blood pressure

Blurred vision, chest pain or discomfort, headache. lightheadedness, dizziness, or fainting, nervousness, pounding in the ears, slow, fast, or irregular heartbeat, unusual tiredness or weakness.

monitor BP continuously if possible or every couple of minutes, double check all concentrations with additional nurse, may result in rebound hypotension due to tissue ischemia when discontinued, monitor EKG and CVP,if patient is awake instruct them to report headaches, dizziness, or chest pain.

IV

Epinephrine

emergency treatment of type I allergic reactions, including anaphylaxis. It is also used to increase mean arterial blood pressure in adult patients with hypotension associated with septic shock.

induces increased vascular smooth muscle contraction, pupillary dilator muscle contraction, and intestinal sphincter muscle contraction.

tachycardia, hypertension, headache, anxiety, apprehension, palpitations, diaphoresis, nausea, vomiting, weakness, and tremors.

heart rate, cardiac rhythm, and blood pressure frequently if an IV drip is to be infused

IV

Medication: What is it for? What is its action? Side Effects Nursing Implications (Vital signs, labs, assessments)

Administration Techniques (example: IV push)

conduction velocity in the ventricles.

Succinylcholine

facilitate tracheal intubation, and to provide skeletal muscle relaxation during surgery or mechanical ventilation

A

depolarizing neuromuscula r blocking agent, succinylcholine adheres to post-synaptic cholinergic receptors of the motor endplate

allergic reactions like skin rash, itching or hives, swelling of the face, lips, or tongue, breathing problems.

Assess respirations, SPO2, mentation

IV push

Vecuronium

Facilitates endotracheal intubation (Paralytic)

relax muscles or as an adjunct in general anesthesia during surgical procedures

Prolonged paralysis ,Bronchospasm/respiratory depression, Apnea

Monitor ECG, heart rate, and BP throughout administration. Observe for residual muscle weakness and respiratory distress during the recovery period. Monitor infusion site frequently.

IV Push

Nicardipine

for the treatment of hypertension, including hypertensive urgency and hypertensive emergency

lowers blood pressure by relaxing the blood vessels so the heart does not have to pump as hard

Headache, upset stomach, dizziness or lightheadedness, excessive tiredness, flushing.

Monitor BP carefully during titration period

IV

Sugammadex

for the reversal of neuromuscular blockade induced by rocuronium bromide and vecuronium bromide in adults and pediatric patients aged 2 years and older undergoing surgery.

creates a concentration gradient which favors movement of rocurionium from the neuromuscular junction into the plasma, which quickly reverses rocuronium- induced neuromuscular blockade.

vomiting, dry mouth, tachycardia, dizziness and hypotension

Monitor ECG, heart rate, and BP throughout administration. Observe the patient for residual muscle weakness and respiratory distress during the recovery period

IV Push

Medication: What is it for? What is its action? Side Effects Nursing Implications (Vital signs, labs, assessments)

Administration Techniques (example: IV push)

Alteplase

indications for alteplase include pulmonary embolism, myocardia l infarction with ST- segment elevation (STEMI), ischemic stroke when given within 3 hours of the start of symptoms

converts plasminogen to the proteolytic enzyme plasmin, which lyses fibrin as well as fibrinogen

Bleeding from puncture sites and wounds, bleeding gums, coughing up blood, difficulty with breathing or swallowing, dizziness, headache .

closely monitored for signs of bleeding or hemorrhage.

IV

Milrinone

cardiac support in patients with acute heart failure, pulmonary hypertension, or chronic heart failure.

inhibits the degradation of cyclic adenosine monophosphate (cAMP)

chest pain; a light-headed feeling, like you might pass out; or. low potassium level-- leg cramps, constipation, irregular heartbeats, fluttering in your chest, increased thirst or urination

Fluid and electrolyte changes and renal function should be carefully monitored during therapy with milrinone. Improvement in cardiac output with resultant diuresis may necessitate a reduction in the dose of diuretic.
