1 INTERVENTIONS TO PREVENT PRESSURE INJURIES:

∞ Avoid Massage over bony prominences.

∞ Protect skin from moisture; use barrier creams, change soiled linens/clothes ASAP

∞ Minimize friction & shear; use draw sheets, proper rotating/positioning techniques (Every 2

hours) and use padding.

∞ Use mattresses, positioning devices, pillows, and heel protectors as deemed necessary. Float the

Heels from the bed.

∞ Assess the Skin Daily; Clean Skin Routinely; Mild cleansing agent, NO hot water.

2 ARE MONTGOMERY STRAPS?

∞ Montgomery Strips are adhesive strips (TAPE) applied to the skin on either side of the surgical

wound to avoid irritation to the skin near the incisional area to minimize skin break down to the areas.

∞ The adhesive strips have holes for using gauze to tie the dressing securely.

∞ When the dressing is changed, the ties are released, the dressing replaced, and the ties secured

again without removing the adhesive strips.

3.CLOSED DRAIN SYSTEM (Hemovac or Jackson-Pratt)

∞ A closed system uses a vacuum system to withdraw fluids and collects the drainage into a

reservoir.

∞ Must be emptied and measured at least once every shift.

∞ Cleaned by using sterile technique

4.OPEN DRAIN SYSTEM (Penrose)

∞ Drain fluid on to a gauze pad or into a stoma bag.

∞ They are likely to increase the risk of infection.

∞ Drainage of a cavity through an opening in the chest wall into one or more

drainage tubes are inserted.

∞ The opening not being sealed against the entrance of outside air.

5.*Tell me the most important facts that cold therapy helps with

 Reduces bleeding and swelling by cooling the surface of the skin and tissues, ice causes vasoconstriction.

 Reduce muscle spasm. Another benefit of cold therapy is reduced muscle spasm.  Decrease metabolic rate. By reducing the cells metabolic rate, ice reduces the cells oxygen requirements.

6.*Tell me the most important facts that heat therapy helps with

 Improving circulation and blood flow to areas due to increased temperature.  Increasing the temperature of the afflicted area even slightly can soothe discomfort and increase muscle flexibility.  Relax and soothe muscles and heal damaged tissue.

7.*What is the difference between Hydrocolloid dressings and Alginate dressings?

 Hydrocolloid dressing is a type of dressing that provides a moist and insulated healing environment.  Hydrocolloid dressings protect uninfected wounds and allow the body's own enzymes to assist in the healing process.  Alginate dressings, which stimulate platelet aggregation and absorb excess exudate forming a hydrophilic gel over the wound surface  DO NOT USE ALGINATE on very dry wounds because a burning sensation may be experienced as fluid is 'drawn' from the wound bed as a result of the hydrophilic effect.

8.*Name the 3 different types of feedings and what happens with each of the

  1. Nasogastric Tube (NG Tube) soft tube that goes in through the nose, down the throat, and into the stomach. They're used to feed formula to a child who can't get nutrition by mouth. Sometimes, kids get medicine through the tube.
  2. Nasojejunal Tube (NJ Tube) is soft and flexible so it can pass through the nose and stomach. (It carries food through the nose to the jejunum-part of the small intestine)
  3. Percutaneous endoscopic gastrostomy (PEG) A tube inserted through the wall of the abdomen directly into the stomach. It allows air and fluid to leave the stomach and can be used to give drugs and liquids, including liquid food, to the patient.
