IV Medications

3 Types

o IV bolus (Push) o Intermittent infusion (Piggyback) o Continuous infusion in large volume solution

Nurse Responsibilities

o Support positive outcomes o Reduce risk of adverse effects o Integrate med administration into pt plan of care o Provide pt/family education

Safety

o Know rights of med administration o Check medication at least x3 against MAR  As removing med from storage  As preparing med  At pt bedside before administering o Only administer properly labeled meds o Perform accurate dosage calculations o Know side effects o Know desired effects o Know antidote if one available

************************ IV Medication compatibility is very important*********************

o Minimize incompatibility of IB bolus by flushing line with 10 mL or 0% sodium chloride before and after med administration. o Any precipitate, haze, color change or gas evolution discoloration change meds are incompatible.

IV Rates Calculation

Do’s before IV medication Admin

o Take vital signs o BP o Heart Rate o Respirations o Pain score before giving analgesics

Pt teaching o Instruct pt to report symptoms of pain, burning, itching, swelling or any reactions to the medication.

IV medication Administration

o IV bolus Admin:  Concentrated meds  Small amount of fluid needed  IV placement needs to be confirmed before administration

o Intermittent IV Bolus Admin:  Intermittent delivery administration throughout the day  Primary line with maintenance fluid required  Primary fluid must be positioned lower than piggyback.

o IV Bolus (IV Push) Admin:  Delivered over 3-5 min  Must check IV compatibility with med  Saline lock IV must be flushed with 0% sodium chloride before/after med administration.

Central Venous Devices (CVADS) & Peripherally Inserted Central Catheters (PICCS)

o Frequent/ recurrent blood sampling for labs o Delivery of vasoactive meds

o Following med admin flush extension tubing with 10 mL of 0% sodium chloride.
