Gemfibrozil (Lopid) Fibrates

Activates peroxisome proliferator-activated receptor (PPAR). This increases production of LPL and decreased production of triglycerides. Also increases production of lipoproteins, apolipoproteins A1 & A2 which increases levels of HDL.

Available in oral form only. Taken BID 30 minutes before meals.

Statin anti-lipemic greatly increases risk for myopathy Warfarin with fibrates increases risk for bleeding

Do not take if pregnant or breastfeeding

Can cause nausea, abdominal pain, diarrhea. Increase risk of gallstones. Myopathy (pain in muscles) Can cause liver toxicity

Reduces plasma triglycerides Increased HDL cholesterol

Monitor for myopathy, GI symptoms, gall bladder disease. Measure CPK if muscle pain occurs. Monitor liver function

Triglycerides numbers are reduces and HDL cholesterol has increased

Report GI symptoms, inability to tolerate fried foods, bloating, muscle/joint pain, jaundice, and fatigue to PCP
