NSG Ch. 4 - Clinical Decision Making

Clinical Judgment - Critical thinking = using your knowledge and provide the clearest picture possible, thinking in a logical manner - includes reflection - Clinical decision making = separates nurses from other assistive personnel - We observe and ask questions - We clarify where the patient is

  • Clinical Judgment
    • Essential in the nursing process
    • Improves patient outcomes
    • Explains the Why of what we do in Nursing Model: Critical Thinking “specific knowledge base” (Knowledge, Environment, Experience) -> Nursing Process -> Clinical Judgment → Clinical Decision making (based on patient needs)

ADPIE: Assessment, Diagnosis, Planning (goals and outcomes), Implementation, and Evaluation = critical thinking -> clinical decision making

  • Steps of the Nursing Process

General Critical thinking: - Scientific method, problem solving, and decision making

Specific Critical thinking: - Diagnostic reasoning & inference - Clinical decision making - Deductive reasoning: you have little information and no specifics when forming a conclusion - Inductive reasoning: you have specific information to form a general conclusion

Critical Thinking Model: to make clinical decisions - Competence - developing - Specific Knowledge Base - Experience

  • Attitude

Reflection: - Reflection is important because it helps you discover the purpose and meaning of your decision making - Improves your problem solving ability - Do reflection before you leave the workplace - NOT INTUITIVE

Critical Thinking Attitudes: use these in your practice - What are you good at and what can you improve on?

  • Confidence
  • Integrity
  • Risk Taking
  • Fairness
  • Discipline
  • Strength: Perseverance and Creativity
  • Weakness: Confidence

Nursing Process: is competency - meeting patient needs - Putting the patient first - Requires critical thinking - Follows a logical sequence - Framework for responsibility and accountability - ADPIE - continuous cycle of thought and action 1. Assessment - collecting patient data 2. Diagnosis - consultation and managing health needs 3. Planning - transferring data to make a patient plan 4. Implementation - implementing the plan 5. Evaluation - did our plan work and what do we need to adjust

  • Data collection: must by systematic and continuous, and should include all of the information about the patient
  • Sources of data: client, support people, nurse’s experience, and patient records
