ACTIVE LEARNING TEMPLATE: Nursing Skill

-Tracheal stenosis: tracheal lumen narrows due

to scarring.

-Tracheal wall necrosis: tissue damage from

inflated cuff pressure.

-Fluid retention, -Oxygen toxicity, -Aspiration.

-Accidental decannulation.

PRE: gather proper equipment and supplies before

procedure. Assess tracheostomy. Hyper oxygenate before

and after and during. Suction patient. Document findings

before procedure. INTRA: Assess respiratory q1-2 hours.

Monitor vitals. Clean stoma site. Assess cuff pressure.

POST: provide trach care q8 hours. Suction when necessary.

Administer pain meds, antibiotics, adequate nutrition.

Oral care q2 hours.

Ensure patient has adequate oxygen supply via

Tracheostomy vent.

- Sterile surgical incision into the trachea the skin and

muscles for the purpose of establishing an airway.

Assess lung sounds, oxygen saturation level,

respiratory status, & pain level prior to suctioning.

Changing tracheostomy dressing and

replacing/cleaning inner cannula. Make sure to do

tracheostomy care following sterile technique.

Monitor/ assess tracheostomy every 1 to 2 hours.

Reposition q2 hrs. monitor I&O, daily weights, monitor

for respiratory.

- Educate patient on the proper cleaning and

saturation, lung sounds, and effort.

-Establish a method of communication for the client

with staff.

-provide adequate humidification and hydration to

reduce mucus plugs and thin mucus.

Inner cannula needs to be replaced. Clean stoma. Suction trach. Replace tracheostomy tie.
