Knowledge Assignment: Evidence Based Skin Care After this week’s reading, I have learned how important it is to understand evidence-based skin care. I learned that preventing skin breakdown and pressure ulcers I one hundred percent preventable if you stick to evidence based practice. Preventing pressure ulcers in hospitals requires an interdisciplinary approach of care. Every year in the United Stated more than two million people develop pressure ulcers. Many do not understand the seriousness of pressure ulcers like infection, pain, and it leading to possible sepsis. In the hospital setting there are many ways we can prevent skin breakdown and pressure ulcers. It is important for us to recognize our patients who are bed bound must be changing positions often. If a patient cannot change positions on their own, we must reposition them every two hours. No nurse can do this on their own so we must work as a team to make sure the patient is getting repositioned every two hours. We must as nursing make sure we are assessing a patient’s skin on common areas like the coccyx, heels of feet, back of the head, and elbows. If we see the start of a pressure sore, we need to put barriers to prevent the pressure sore from possibly getting worse. If a patient has more than stage one pressure injury, we must investigate a special bed. We must think of other ways to prevent pressure ulcers like massaging the boney areas to help with circulation. Other ways a patient may experience skin breakdown are our patients who are incontinent. Making sure our patients are never sitting in their urine or feces. Having the knowledge that a patient is incontinent we must check every hour to see if the patient needs to be cleaned up. When cleaning up the patient we must use warm water and soap to get all the urine and feces off the skin. If the patient is showing signs of skin breakdown, we need to use skin barriers line zinc oxide. This will help prevent skin breakdown, redness, and bleeding. Registered nurses are very busy depending on how many patients they have and how sick the patients are. We can use certified nursing assistants to help us make sure patients are getting the care they need. This would include planning of action where the patient will be turned every two hours, changed when needed.
