Dementia patients often present with behavioural and psychological symptoms (BPSD) such as aggression, wandering, agitation and anxiety (Magierski and Sobow et al, 2020). These symptoms, also known as ‘responsive behaviours’ affect up to 80% of people with dementia (Kirkham and Sherman et al, 2017). These symptoms are distressing for those with dementia and caregivers, and up to 50% of cases will be controlled with antipsychotic medication (Tampi and Tampi et al, 2016). In recent years the use of these medications for BPSD has come under review due to their potential for significant harm and side effects, and lack of effectiveness in symptom control (Bjerre and Farrell et al, 2018). However, despite the fact that the risks out-weigh the benefits in people with dementia, they continue to be used (Magierski and Sobow et al, 2020). I argue that they should not be used as first-line options, but instead as a last resort.
These medications have the potential for considerable harm in the elderly population showing an increase in the overall risk of death, cerebrovascular adverse events, gait disturbances, falls and somnolence (Tampi and Deena et al, 2016). Furthermore, these medications may actually contribute to worsening symptoms of dementia (Magierski and Sobow et al, 2020). According to this information, it is time to find better solutions in dealing with behavioural related issues in people with dementia.
Evidence now suggests that these behaviours may be an expression of ‘unmet’ needs, and that personalised non-pharmacological management may be one solution. In one meta-analysis it was shown that non-pharmacological interventions such as music therapy, and interventions delivered by caregivers had an effect equal to that of antipsychotic medication (Wang and Albayrak, 2018). Thus, in conclusion I would reinforce the point that antipsychotic medications should be reserved as a last resort to treat behavioural and psychological symptoms of dementia, given the high risk of side effects, limited efficacy and emerging non-pharmacological alternatives.
