Total Hip Replacement ( THR)

A THR use two- device consisting of an acetabular cup that is inserted into the acetabulum and a femoral component that is inserted into the femur to replace the femoral head and neck.

Pre operative care

❖ Explain procedure to the patient. ❖ Assess the neurovascular starus(circulation,sensation,mobility) ❖ A surgeon frequently order a prophylactic antibiotic preoperatively to minimize the chance of an infection. ❖ The patient donates blood before surgery per guidelines. ❖ Some hospitals have joint camp programs where group of patient undergoing joint replacement are admitted on same day. Post operative care. ❖ Provide general post operative care ❖ Prevent complications.

Complications of THR

❖ Hip dislocation ❖ Skin breakdown ❖ Infection ❖ Bleeding ❖ Pain ❖ Thromboembolic complication(DVT,PE,TE)

Nursing intervention

  • Ensure hip that the hip is not allowed to become adducted. Use triangular pillow or pillows
  • Turning patient requires abductor pillow to remain in place. Turn patient as a whole. Not allowing hip or leg to fall forward or backward. Use pillows to support raised limb.
  • Monitor for skin integrity of opposite heel
  • Ensure limb remain in abduction when moving patient out of bed.
  • Prevent postoperative pneumonia by encouraging deep breathing and coughing and use of incentive spirometer.
  • Pain control is of the utmost importance. Provide regularly scheduled analgesics and ensure break through analgesia is provided prn. Decrease pain allows for earlier mobilization and less complications of immobility.
  • Monitor level of consciousness and orientation. Many older patients have alternation in their mental status after surgery due to anaesthetics, analgesics, blood loss, and environmental changes
