FAMILY BACKGROUND

- In Puno’s Family, the family name is Lany Puno 29 years old, she is a housewife, and her husband is Ricky Puno. They live in

Purok Uno, Barangay Tibig, Lipa City. They can speak Tagalog and Bisaya and they’re both Roman Catholic. They are nuclear

in the family structure even though they don’t have children yet. The length of residency is about 2-6 years. The fatheracts as a

security guard for the family, gaining an increasing income of Php 10, 000 pesos weekly. Lany’s father is dead but her mother

is still alive name Salve Esparraguerra, 55 years old.

- In Health Care Practices, the husband and wife are following self-medication, if they have health concerns, they consult to

doctor, and if non-related health problems, they are consulting to the family. They’re partially immunized with COVID-

vaccines. In Housing, the type is strong, ownership is rented, as well as the ventilation and lightning facilities are satisfactory.

- Going on, in the environmental indices, their water is Level III which is a waterworks system or individual house connection,

they get drinking water through commercially distilled water, they store their water in a covered container and then the toilet is

water sealed.

- Moving on to garbage disposal, the family store the garbage can with a cover and a plastic garbage bag, and dispose of itin

garbage truck collection. The Drainage is a covered canal, and clean it every day. In the environment, the type of plants is

herbal and they do not have air pollution problems in their area. Sanitary food handling and preparation, they prepared washed

and stored in the refrigerator, cooked foods on the stove, bought mostly at the public market, and most importantly washed

their hands before and after preparing food and eating.

- Based on my interview, Lany’s mother experienced hypertension and consult the physician when she experienced signsand

symptoms.

Hypertension A health deficit was found to be the first level assessment while

a. Inability to make decisions concerning taking appropriate health actions due to failure to comprehend the nature of the problem.

b. Inability to provide adequate nursing care to the sick, disabled, dependent, or vulnerable/at-risk family member due to inadequate knowledge about the health condition and skill in carrying out the necessary nursing care.

After the nursing intervention, the family can understand the diseases and treatment plan. The family can also take action to help improve the situation of the member who experiences health problems. The client’s preference for care is also considered in the decision-making. These interventions are specified based on the health condition:

a. Ways to appropriately cope, or control blood pressure, adhere to the therapeutic intervention, lifestyle modifications, and prevention of changes.

After days of nursing interventions, the family will: -To know the health condition and skill in carrying out the necessary nursing care needed by the client.

-To have sufficient information regarding appropriate sustenance that will improve and decrease hypertension and avoid the existence of relative problems in the potential.

-To follow the correct routine with concerns about diet and physical condition. -To provide specific medications for the health problem.

Promotive Interventions

  1. Expand the education of the family on methods to properly cope with hypertension and to stimulate well-being, particularly: a. Educate the patient to cease stressful conditions and exhausting physical exercise. b. Provide a quiet environment, and explain therapeutic management. c. Discuss with the family the signs and symptoms indicating the onset of hypertension. d. Discuss with the family the risk considerations of hypertension which includes family history, age, salt intake as well as alcohol intake. e. Educate the family about accessible therapy choices.

Occasional Hospital visits/check-ups and Virtual Home visits/ Video Conference (Health teaching/Health promotion)

The materials or resources required include the following:

  • Brochures/infograp hics/Booklets on hypertension

  • Equipment such as sphygmomanometer

  • Resources such as time and effort of the nurse, healthcare professionals, and the family.

Preventive Interventions

  1. Analyze practices to inhibit hypertension or complications with the following:

increasing the continuing skills of the patient through a. Establishing a personalized hypertension action strategy. The plan will detail valuable information such as daily medications, how to manage them, and how to monitor hypertension indications. b. Promote check-ups and require medical appointments with a health physician to minimize hypertension as well as alter risk factors.

  1. Conduct Hypertension Rehabilitation which contains:

a. Physical exercise b. Information and discussion c. Developmental mediation d. Psychosocial treatment e. Nutritional assessment and counseling f. Education, including on the appropriate use of recommended medications
