For the purposes of this case-control study, we would collect the following data:

Within the last five years, how often have you purposely exposed yourself to the sun or used

tanning beds?

How often do you use sunscreen or other forms of skin protection?

Do you tend to get sunburn easily or get freckles after spending time in the sun, and if yes, how

quickly does this occur?

What is your skin type (e.g. fair skin, freckles)?

Do you have any family members who have been diagnosed with melanoma?

4. Other risk factors (at most 5 risk factors) (5 pts)

In addition to the data mentioned above, we would also collect information about the following

potential risk factors for melanoma:

Personal history of previous skin cancer diagnosis

Family history of melanoma and other skin cancer

Immunodeficiency

Outdoor occupation (e.g. farming or construction)

Exposure to chemicals or substances that can increase the risk of skin cancer (e.g. coal tar,

arsenic).
